Community Service Evaluation

The purpose of this questionnaire is to understand your reaction to the group community service program.  We are trying to make this questionnaire as clear as possible, so please let us know if there is any sentence or word that you do not understand.

Instructions: Please select True if you agree and False if you disagree with any of the below statements.  Please specify why you disagree with any of the statements.  Feedback you give will only be used to improve our program and will not affect your status in the group or in the future.

  1.    I believe the supervisors were helpful leaders to the group through

                           service projects and activities.

  2.    I believe that the supervisors encouraged me to make some decisions.

  3.    I believe the supervisors were active participants in completing the

                           community service projects.

  4.    I could understand the words and terms used by the supervisors.

  5.    I felt free to talk during the service projects and activities. 

  6.    I believe the supervisors tried to understand my feelings and concerns.

  7.    If I was interested in "volunteering," not court ordered, I would contact the

                           Office on Youth to assist in finding a project for me.

  8.    I believe that my involvement in the group will help me to stay out, or

                           discourage actions which may involve me in the courts or this program in

                           the future.

  9.    I do not feel that there were enough projects to participate in.

10.    I feel that I made a difference during the community service program.

11.    I knew what was expected from me for the various community service

                            projects.

12.    I believe the supervisors made me feel comfortable and safe in the

                            group.

13.    I believe that other community service participants gave as much an effort in working

                           as I did.

14.    I believe that the supervisors gave as much of an effort working as I did.

15.    I believe that our group projects got our group to work together as a

                            team to finish the project.

16.    I believe that the group community service programs are a good

                            program for juveniles to complete their community service hours.
